Planning For Your Eye Exam
Whether you are a new or returning patient we want to take this opportunity to thank you for choosing Nowlan & Moore Optometric.
If this is your first Eye Exam we look forward to meeting you, and kindly ask that you arrive a few minutes early to allow extra time to complete forms. In addition, please take some time to review and answer the following questions. Your answers will greatly help Dr. Moore and Dr. Mazur assess your risk of possible eye disease and vision problems.
- What previous overall health problems have you experienced?
- Do you take any medications, and if so what are they?
- Any historical family vision concerns?
- Anyone in your family have diabetes, high blood pressure, heart disease, or any other health problem?
- Experiencing any specific vision problems that brought you to us?
Our Downtown Winnipeg location is conveniently and easily accessible by foot, bus and automobile. Based on individual requests we can accommodate appointments before, during, and after your normal working hours. If you are driving we suggest parking in either the Hudson Bay or Portage Place parking lots and using the indoor walkway.

Dr. Ross Moore specializes in eye muscle imbalance, macular degeneration, children’s vision therapy and takes great pride in providing you with quality eye health care. As a practicing member of the Doctors of Optometry in Canada and Manitoba we specialize in the examination, diagnosis, treatment, management, and prevention of disease and disorders of your eye, your eye health, and overall vision system.
Even if you don't wear eye glasses, a regular Eye Exam is a preventative measure for not only your eye health care but your overall health.
